The paper enrollment process allows you to print out the contract and mail or fax it in.
It includes three pages: instructions, enrollment information, and contract text.
At the right is a preview of Page 2. The fields with a blue background may be filled out while the PDF is open. This will help maintain legibility and speed up your enrollment process.
Simply fill out as much as you can and print the form. The rest may be completed at your leisure.
The instructions page includes all fax and mailing information.
All forms faxed in between 9-5 CST are typically processed the same day.
